All faxed and mailed in forms, including checks, must be postmarked by Aug. 26, 2011. After Aug. 26, you may register online. Online registration ends Sept. 16, 2011 at 5:00 p.m. central. After Sept. 16, you may register at the event.

Cancellation Policy
Cancellations must be received in writing by Aug. 22, 2011. A processing fee of $100 will be assessed for all cancellations received by Aug. 22, 2011. Send an e-mail cancellation request to insight@showcare.com or fax request to 905.479.9297.

No refunds will apply to cancellations received after Aug. 22, 2011.
