    *To qualify for the group rate, the names of at least five registrants from the same organization must be submitted together to insight@insight-net.org. InSight will provide instructions to complete your group registration. Speakers and other individual discounts do not qualify for the group rate. Group rates are non-refundable.

    If you wish to register for the CIO Forum in conjunction with any of the InSight365: 2013 Annual Conference options above, please first email Pam O'Brien at ciopeers@insight-net.org to begin the registration process.

    Cancellation policy

    Cancellations must be received in writing by August 26, 2013. Group registrations are non-refundable. A processing fee of $100 will be assessed for all individual cancellations received by August 26, 2013. Send an email cancellation request to insight@insight-net.org. No refunds will apply to cancellations received after August 26, 2013.

    Substitutions need to be made in writing and received on or before August 26, 2013. Substitutions onsite will be processed if the attendee presents a letter on organization stationery requesting the substitution and identifying the name of the pre-registered attendee and substitution.
