Horizon Admin-Rx: Overcoming Alert Fatigue
The speaker will present how University Hospital (UH) in Augusta, GA addressed its issue with “alert fatigue.” More than 240,000 doses of medication are documented each month using Horizon AdminRx. Using Clinical Query, UH determined that almost 16 percent of these doses received an alert, the most common alert being “administered too late” (71 percent). Staff focused too much time, energy and worry on these Late Alerts and overlooked the more significant alerts, such as “no med order found” and “incorrect amount.” UH altered its alert settings and expects to see a dramatic decrease in “alert fatigue.”

Automated Chart Auditing the HSM Way: Leveraging Report Automation to Improve Nursing Documentation
HSM is live and data is being collected. You may ask yourself "How can I audit all of these charts – who has the time! What do you do to ensure data integrity?" Crystal Reports Software provides an automated framework to verify data integrity, regulatory compliance and documentation feedback real-time. Mercy developed a report that allows each Perioperative phase to accumulate knowledge about common charting errors and checks the data integrity for every case. Any data-mining error or regulatory requirement is added to a collective report that reviews these errors for each nurse. The end result is improved data collection, improved regulatory compliance, and a satisfied nurse.

Monitoring Horizon Expert Orders Adoption Using an Adoption Dashboard
Peninsula Regional purchased Horizon Expert Orders (computerized physician order entry-CPOE) to help improve the quality and consistency of patient care. In order to monitor the rate of adoption of HEO by PRMC physicians and identify any issues in adoption by physician specialty, physician, nursing station, or other variable, PRMC, in connection with McKesson identified a list of needed information to be monitored, tracked and trended.
These data have been installed in Horizon Business Insight (HBI) as an HEO Adoption Dashboard. This dashboard includes the major metrics regarding adoption levels and is backed up by HBI Highlights with detail to identify where adoption levels can be improved. This presentation will include a review of the data on the dashboard and a discussion of how the data are used to evaluate educational needs of physicians, required system build modifications, and other opportunities for improving use of the system.

Integrating Clinical and Automation Solutions to Drive Nursing Activities to the Point of Care
The presentation will focus on the journey that Holzer Medical Center began in 1996 with the goal of integrating technologies so that nursing care is provided safely and effectively at the bedside. This journey is interdisciplinary in nature as nursing care can only be provided when all who support care are involved. Through a partnership with McKesson, HMC has implemented several clinical and automation solutions that have moved pharmacists out of the pharmacy and to the bedside; provided just in time medication administration with safeguards built in the protect the last line of defense for an error, the nurse; and expanded computerized documentation of care so that it integrates with other technologies, thereby reducing the time spent by nurses repeating information documentation. You will also learn of upcoming strategies that will be implemented within the next few years that will continue to foster a safe patient care environment.
